Fillable Priority Reservation Form from the American Biographical Institute Item Info

A typewritten letter from the American Biographical Institute to inform Crabtree that the editing of his biographical profile for “Personalities of the West and Midwest” was done. Crabtree was entitled to a pre-publication offer where he could get a plaque, as well reserve copies for himself and others. On the opposite side is a fillable reservation form for copies of the publication and award certificate. There are two envelopes, one addressed to the American Biographical Institute and one addressed to Crabtree. The latter has names, times, and notes that don't seem to have any correlation to the item.
